About the position

The RN Case Manager (CM) at UC San Diego Health will work in the Care Connections Hub, primarily handling post-discharge calls from patients who have recently been discharged from a hospital or skilled nursing facility. This role is crucial in ensuring patients understand their post-discharge instructions, medication reviews, and appointment confirmations, while also addressing any concerns that may lead to avoidable readmissions. The position operates in a hybrid model, requiring both remote and on-site work, particularly during the training period.

Responsibilities

  • Handle post-discharge calls from patients to ensure understanding of discharge instructions.
  • Review medications with patients and answer any questions they may have.
  • Confirm that patients have appointments set and can attend them.
  • Conduct difficult phone conversations and escalate issues as necessary to prevent readmissions.
  • Work collaboratively with the Care Connections Hub team to provide support to patients post-discharge.

Requirements

  • Must be a graduate from an accredited school of Nursing.
  • Registered Nurse (RN) license issued by the state of California.
  • Prior case management experience in a large acute care or community setting and/or utilization review experience in a hospital or IPA setting.
  • Experience with MCG and/or InterQual guidelines.
  • Strong teamwork and interpersonal skills.
  • Proficiency in Information Systems and computer programs including MS Office (Word, Excel, Outlook, PowerPoint, etc.).

Nice-to-haves

  • Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN).
  • Accredited Case Manager (ACM) and/or Certified Case Manager (CCM) certification.
  • Prior experience with referral processing, utilization review, and discharge planning.
  • Proficiency in Electronic Health Records (EHR), preferably Epic.
  • Three to five years of clinical RN experience in acute care or post-acute care/community settings.
