Case Manager (Foster Care)

About the position

The Case Manager (Foster Care) at UCAN is responsible for providing direct individual and family casework services aimed at facilitating healing through trauma-informed care and positive youth development. This role involves assessing client needs, developing service plans, and coordinating resources to support youth and families in achieving self-sufficiency and permanency.

Responsibilities

• Completes assessments and service plans for assigned clients that address their unique service needs.
• Implements and monitors client service plans, reviewing progress with supervisors and DCFS as required.
• Assesses and monitors the safety of clients in their living arrangements and makes recommendations for appropriate placements.
• Develops knowledge of community resources and maintains productive relationships with them for client support.
• Provides referrals and linkages to appropriate resources, securing written agreements as needed.
• Creates organized plans to ensure all casework tasks are completed on time.
• Maintains timely communication regarding clients through written correspondence, emails, and phone calls.
• Coordinates client services with courts, DCFS, and other service providers or community organizations.
• Maintains accurate and timely client records and reports progress to supervisors and in court proceedings.
• Engages clients and their families through outreach and maintains required face-to-face contact.
• Completes parenting assessments and provides education to teen parents on child development and health issues.
• Models appropriate adult/child interactions and assists clients in accessing necessary services and resources.

Requirements

• Bachelor's degree in Human Services or related field required.
• 0-2 years of experience in human services, child welfare, or crisis intervention.
• Licensed Child Welfare Services Employee or license eligible.
