The Staff Registered Nurse (RN) or Licensed Practical Nurse (LPN) provides professional and comprehensive nursing care within the scope of agency services, specifically targeting patients requiring skilled clinical home care services. This position is eligible for hybrid and flexible schedules, allowing for a balance between in-home patient care and administrative responsibilities. The role involves regular assessments of patients' needs, initiating and adapting care plans, and providing nursing care through ongoing visits to a group of patients. The RN/LPN will coordinate care with other healthcare professionals, ensuring that all aspects of the patient's care are addressed and communicated effectively. In this position, the nurse will be responsible for notifying physicians of any changes in patient conditions, obtaining new orders as necessary, and supervising Licensed Practical Nurses and Home Health Aides. The nurse will also function as a case manager for assigned patients, implementing preventative and rehabilitative nursing procedures while utilizing community and agency resources to optimize patient care. Documentation of services provided is crucial, and the nurse will maintain patient records in accordance with agency standards. The role requires participation in interdisciplinary assessments, development and evaluation of care plans in partnership with patients and their caregivers, and providing education and counseling to patients and families. The nurse will also be involved in quality assessment and performance improvement programs, ensuring compliance with agency policies and procedures.
