About The Position

The On Call Nurse at Infinity Hospice Care is responsible for providing necessary hospice care to patients and families during non-business hours. This role emphasizes a commitment to comfort care and the hospice philosophy, ensuring that patient and family needs are met with compassion and professionalism. The position requires collaboration with the Director of Patient Care Services and the interdisciplinary team to manage after-hour calls, admissions, and nursing visits, while maintaining compliance with company policies and procedures.

Requirements

• Registered Nurse from an accredited program currently licensed and in good standing with the State Board of Nursing in the state of licensure.
• At least 1 to 2 years experience in nursing in an acute care setting or 1 year experience in home health, hospice, or comparable experience is preferred.
• Desire and aptitude for the hospice philosophy and comfort care.

Responsibilities

• Respond to patient/family needs and provide necessary hospice care during non-business hours.
• Triage all after-hour calls/admissions/referrals and explain legalities while obtaining informed consent as needed.
• Participate in making after-hour nursing visits to evaluate and assess a patient's condition.
• Make scheduled weekend visits as deemed necessary by the Director of Patient Care Services (DPCS) or RN Case Manager.
• Document all telephone contacts and submit Call Log to the DPCS each Monday morning.
• Attend all home deaths and contact the Primary Care Physician (PCP) after death has been assessed.
• Assist with post-mortem care as required.
• Carry a case load during high census or low staff availability when possible.
• Participate in Interdisciplinary Team meetings and update the Plan of Care for assigned patients based on identified problems and needs.
• Ensure all services provided are in accordance with the patient's Plan of Care.
• Educate patients and families on the disease, disease process, and expectations related to the disease.
• Assist with Continuous Quality Improvement (CQI) activities.
