The Afterhours AIM (Advanced Illness Management) Program is seeking a full-time Registered Nurse (RN) to work on a 7 on/7 off schedule. This position reports directly to the Director of AIM and is responsible for identifying and coordinating patient and family needs while providing preventive and supportive care to palliative patients and their families. The RN will utilize assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life. This role includes afterhours coverage for designated teams and functions as an interdisciplinary advanced illness management support, providing telephonic, video, and in-person interventions after hours. As a Registered Nurse, you will be expected to triage patient needs over the phone until an in-person visit can be arranged, respond urgently to crisis calls from patients and families after hours, and conduct initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral. You will identify the physical, psychosocial, emotional, and environmental needs of patients and families, collaborating with them, attending physicians, and other members of the interdisciplinary group (IDG) to provide comprehensive care. Timely documentation of problems, goals, interventions, and patient/family responses to palliative care is essential, as is effective communication with the AIM team, clinical supervisors, and the patient's primary care physician regarding any changes in patient status or needs during afterhours calls. Additionally, you will assist in overseeing the utilization of medications, durable medical equipment (DME), and supplies related to the care plan, ensuring alignment with patient needs and physician orders.
