Responsibilities

  • Collect and record patient care data following the guidelines of patient care services, regulatory agencies, and third-party payers.
  • Prepare for evaluations by obtaining patient history and demonstrating knowledge of patient diagnoses.
  • Select and administer tests and measures, synthesizing evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
  • Establish treatment plans based on evaluation results and best practices.
  • Collaborate with interdisciplinary teams, patients, and their support systems.
  • Assess and re-assess the need for modifications to plans of care and goals.
  • Identify the need for consultations and refer patients to other healthcare professionals or services as necessary.
  • Initiate and complete discharge plans, providing education and training to patients and their support systems.
  • Provide functional training, including activities of daily living (ADLs), therapeutic activities, and rehabilitation equipment prescription and training.
  • Administer safe and effective interventions in compliance with JAG Physical Therapy standards, including therapeutic exercises and modalities.
  • Document all treatments provided using WEB PT EMR and present facility-level in-services annually.

Requirements

  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements in the appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience or knowledge with EMR software is preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and independently with minimal supervision.
  • Maintain a positive, professional demeanor at all times.

Nice-to-haves

  • Experience with EMR software is preferred.
  • Knowledge of various therapeutic techniques and modalities.
