About the position

Open Sky's North Star, Behavioral Health- Justice-Involved program is a new model of care designed to assist MassHealth Members who are Justice-Involved, either as current inmates, detainees or on probation or parole. The goal of the program is to support individuals in navigating and successfully engaging with health care services, with an emphasis on behavioral health Services. The program aims to improve health outcomes and decreasing fatal overdoses while providing connections to health care and community services. The Lead Navigator - Clinical Support, under the direction of the Division and Clinical Director, will provide In-Reach and Re-entry supports, assessment, education, support and safety planning for individuals enrolled in the Community Supports Program - Justice Involvement division. They also train and mentor navigators, serving as a point of contact for integrated care coordination and care transitions in collaboration with individuals enrolled, justice agencies, medical and behavioral health providers, pharmacies, various social service agencies and other agencies and supports as indicated.

Responsibilities

  • Implement interventions as identified in the Support Plan and document Member progress to maintain a permanent record of activity and services provided.
  • Act as a liaison between members, community agencies, justice entities and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
  • Provide and coordinate crisis response supports to members as necessary.
  • Communicate all potential risk-related and other pertinent information to program leadership according to established program and Agency procedures.
  • Participate in the Emergency On-Call rotation.

Requirements

  • High School Diploma, GED or equivalent, required.
  • Bachelor's degree in social work, psychology, or a related field preferred.
  • Minimum of 2 years' experience working with homeless, justice involved, adults with serious and persistent mental illness, substance use disorders, and underserved populations, required.
  • Valid Drivers License, acceptable driving history and reliable transportation, required.
  • Experience with local resources, navigating health, behavioral health and/or substance abuse treatment systems strongly preferred.
