About the position

The Care Coordinator plays a vital role in the Care Team by being one of the first points of contact for clients and caregivers. This position focuses on creating a caring culture through exceptional service, optimizing scheduling, and ensuring client satisfaction and safety. The Care Coordinator is responsible for building strong relationships with caregivers and assisting in the collaborative coordination of care to enhance efficiency and reduce waste.

Responsibilities

  • Accept and coordinate referrals, client welcome calls, and data entry into EHR.
  • Communicate effectively with referral sources to resolve client registration or authorization issues.
  • Maintain clients' EHR by entering new information timely and accurately.
  • Address client concerns with empathy and in a timely manner.
  • Support other Care Team members to provide excellent customer service and foster a collaborative environment.
  • Recruit and onboard caregivers, including conducting interviews and coordinating training.
  • Communicate with case managers and practitioners to facilitate client services and ensure continuity of care.
  • Monitor and manage caregiver master schedules.
  • Assist with paperwork flow, ensuring accuracy and completeness.
  • Handle incoming and outgoing calls professionally and kindly.
  • Monitor usage of Electronic Visit Verification (EVV) and enter client visits accurately.
  • Generate timesheets and ensure timely transmission to payroll.
  • Compute, record, and submit initial billing, and conduct research on outstanding billings.
  • May reconcile accounts receivable and accounts payable reports.
  • Generate reports for the Administrator and Care Team.
  • Follow company policies and procedures with integrity.

Requirements

  • High School Diploma or GED
  • 2 years of relevant work experience
  • Bilingual in English and Spanish
  • Excellent organizational skills
  • Effective verbal and written communication skills
  • High attention to detail and accuracy
  • Proficient in Microsoft Office applications
  • Ability to learn job-specific software systems

Nice-to-haves

  • Experience with EHRs and Electronic Visit Verification (EVV)
  • Local knowledge about community health care and social welfare resources
