About the position
The Care Manager Extender will support care managers in delivering Tailored Care Management and fulfilling member contact requirements. This role aims to enhance the efficiency of care teams, allowing care managers to focus on key tasks and provide additional time for members with complex needs. Extenders will work closely with care managers and the care team to support members holistically and document interactions according to departmental standards.
Responsibilities
- Perform general outreach, engagement, and follow-up with members.
- Coordinate services and appointments, including wellness reminders and transportation arrangements.
- Engage in health promotion activities and provide education on chronic conditions and self-management skills.
- Share information with care managers and the care team regarding member circumstances.
- Provide and track referrals and assist in obtaining community-based resources and social support services.
- Participate in case conferences and support care managers in assessing unmet health-related resource needs.
- Perform other duties as assigned.
Requirements
- High school diploma or GED is required.
- Must be at least 18 years of age and trained in Tailored Care Management.
- Must have lived experience with I/DD or TBI, or be a Certified Peer Support Specialist, or have two years of direct experience navigating the Medicaid delivery system.
Nice-to-haves
- Certified PSS (Peer Support Specialist) and/or WRAP (Wellness Recovery Action Plan) Facilitators preferred.
