About the position

The Care Transition Coordinator at Enhabit Home Health & Hospice is responsible for assisting patients in navigating post-acute care, ensuring a safe transition from acute care to home health or hospice services. This role integrates clinical guidelines into patient-centered transition plans, promotes adherence to these plans, and represents Enhabit in strategic relationships with health systems and physician groups. The position requires a strong focus on quality assurance and achieving referral and admission goals while positively impacting patient outcomes.

Responsibilities

  • Assist patients in the process of navigating post-acute care.
  • Assess, plan, implement, coordinate, monitor, and evaluate options and services for safe transitions from acute care to home health or hospice services.
  • Integrate evidence-based clinical guidelines and protocols in the development of patient-centered transition plans.
  • Promote adherence to post-acute plans and ensure ordered services are completed.
  • Represent Enhabit in transitional care activities and strategic relationships with health systems, hospitals, and physician groups.
  • Monitor execution of transitional care services through ongoing quality assurance visits with referral sources.
  • Meet and/or exceed referral and admission goals.
  • Act as a clinical liaison responsible for care transitions program admission activity for the territory.

Requirements

  • Registered Nurse (RN) or Physical Therapist (PT) license in the state of employment.
  • Minimum of 2-3 years of field experience in home health or hospice care.
  • Strong understanding of customer and market dynamics, as well as transitional care best practices.
  • Good understanding of Federal, State, and local laws and regulatory guidelines governing home health and hospice operations.
  • Excellent communication skills and ability to interact well with diverse individuals.
  • Experience with territory management and strong presentation skills.

Nice-to-haves
