About the position
The Care Transitions Coordinator at Enhabit Home Health & Hospice plays a crucial role in assisting patients with navigating post-acute care. This position aims to enhance patient outcomes and satisfaction by developing patient-centered transition plans based on evidence-based clinical guidelines. The coordinator will also represent the organization in strategic relationships with healthcare systems and professionals, ensuring a collaborative approach to patient care.
Responsibilities
- Assist patients in navigating post-acute care to improve outcomes and referral source satisfaction.
- Integrate evidence-based clinical guidelines and protocols in developing patient-centered transition plans.
- Represent area branches in strategic relationships with health systems, hospitals, and physicians.
Requirements
- Must be a graduate of an approved school of nursing, therapy, or social work.
- Must be licensed in the state where they currently practice.
- Must have two years of demonstrated field experience.
- Must understand federal, state, and local laws governing Medicare certified home health and hospice operations.
- Must have basic technology skills, including operation of a mobile device.
Nice-to-haves
- A registered nurse or physical therapist is preferred.
- Three years of field experience is preferred.
- Previous experience in home health or healthcare sales is preferred.
