About the position

The Case Manager for the Child & Adolescent Clubhouse Program at Highland Rivers Health plays a crucial role in providing treatment and recovery services to youth aged 13 to 18 who are impacted by mental illness and substance use disorders. This position focuses on creating a safe and supportive environment for youth, helping them develop social networks, and coordinating rehabilitation services as part of their individual recovery plans.

Responsibilities

• Assist youth with service needs and develop strategies for supportive social networks.
• Coordinate rehabilitation services as specified in the consumer's individual recovery plan.
• Partner with individuals and CORE services providers to develop individual service plans.
• Assist individuals in transitioning between levels of care and advocate for them with other agencies.
• Provide assistance and support in crisis situations.
• Conduct assessments, monitoring, and provide assistance to individuals.
• Establish professional and ethical relationships with youth, families, and coworkers.
• Provide direct services to individuals and complete necessary paperwork according to policy.
• Manage a caseload ensuring all services are authorized and documentation is complete and current.
• See a caseload at the frequency defined in the service plan and document attempts to contact individuals and ISP reviews.

Requirements

• Experience in case management or a related field.
• Knowledge of mental health and substance use disorders.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with youth and families.
