About the position
The Recovery Manager at CareStar, Inc. is responsible for conducting eligibility evaluations and creating person-centered service plans for individuals applying for specialized recovery programs. This role involves coordinating necessary services, assisting individuals in accessing Medicaid and other resources, and ensuring compliance with legal and program requirements. The position is focused on improving the lives of individuals with behavioral health needs in the community.
Responsibilities
- Conduct face-to-face eligibility evaluations for individuals applying for specific programs.
- Administer the Adult Needs and Strengths Assessments and verify eligibility criteria.
- Develop, monitor, and update individual person-centered care plans.
- Coordinate and authorize all specialized recovery services program services.
- Assist individuals in gaining access to needed Medicaid services and other resources.
- Maintain knowledge of Medicaid, the behavioral health system, and community resources.
- Adhere to CareStar policies and procedures and maintain confidentiality of patient information.
Requirements
- Bachelor's Degree in social work, counseling, psychology, or a related field; or Licensed Social Worker (LSW).
- Minimum of one-year post-degree experience working with individuals with diagnosed chronic conditions or three years' experience with individuals with severe and persistent mental illness.
- Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources.
