About the position

The Case Manager provides case management coordination, individual case planning, monitoring of participants' progress, coordination, and facilitation of progress meetings as well as tracking and reporting individual and program activities. Is accountable for providing comprehensive and excellent services with the goal of achieving optimum outcomes for each participant.

Responsibilities

• Ensure individuals receive ongoing case planning, individualized and personalized goal setting, feedback on progress toward goals, support and follow up for making progress toward individual goals.
• Ensure comprehensive and optimal service delivery for all individuals by serving as the main contact and liaison for the individual and cooperating partners, persons and/or agencies.
• Complete and maintain essential documentation about participant needs and the services provided within established timelines and in accordance with applicable procedures, policies and requirements.

• Proficient in Microsoft Office Suite software and the ability to accurately enter data into computer-based systems.

Nice-to-haves

• Relevant Experience required (1-3 years).
• Associate Degree or Bachelor's degree in a related field preferred.
