About The Position

Requirements

  • Licensed Clinical Social Worker (LCSW) certification is required.
  • 3-5 years of experience in home care, home health, or a related field.
  • Ability to effectively communicate with clients, families, and caregivers in a home environment.
  • Strong focus on personalized care plans and client advocacy.
  • Empathetic and compassionate towards clients and caregivers.
  • Ability to manage logistics of coordinating care across different home settings.
  • Flexibility to respond to the unpredictable nature of home care.
  • Strong problem-solving skills to address issues in a home care context.
  • Ability to analyze data and identify trends to improve care management practices.

  • Regularly assess and adjust client services to ensure appropriate levels of care.
  • Build and maintain relationships with clients and their families as a trusted advisor.
  • Ensure the development and execution of personalized care plans for each client.
  • Oversee client assessments, contract negotiations, and service adjustments.
  • Ensure compliance with the company's standard of care.
  • Lead quality assurance initiatives to maintain excellent service delivery.
  • Resolve client concerns and escalations promptly and effectively.
  • Collaborate with other departments to ensure seamless care delivery.
