About the position

Comunilife Inc. is seeking a dedicated Case Manager for its Life is Precious program located in Long Island, NY. This non-clinical suicide prevention afterschool alternative program is specifically designed for Latina teens aged 12-17. The Case Manager plays a crucial role in supporting these teens and their families by building protective factors that help prevent suicide attempts and ideation. This is achieved through individual, group, and family counseling, as well as outreach, referral, and follow-up services. The Case Manager will work closely with clients to ensure they receive the necessary mental health treatment and encourage their active participation in the Life is Precious program. In this role, the Case Manager will be responsible for engaging with girls referred to the program, providing case management and counseling to connect them with essential services. The position also involves creating age-appropriate programming that expands horizons and provides healing experiences for the clients. Additionally, the Case Manager will reinforce vocational exploration, internships, and community service to enhance participants' career planning and personal growth. Coordinating trips, celebrations, and special events that promote cultural pride and awareness is also a key responsibility. Collaboration with community agencies, public officials, and organizations is essential, as is representing the program to community providers such as schools and churches. The Case Manager will document community activities and family involvement, ensuring a comprehensive approach to support. Responsibilities

  • Provide outreach, engagement, and follow-up with girls referred to the program to assure continuity of mental health treatment and encourage participation in the Life is Precious program.
  • Assist with client recruitment and case management to connect clients with services.
  • Create age-appropriate programming for young adults that expands horizons and provides healing experiences.
  • Reinforce vocational exploration, internships, and community service to enhance participants' career planning and personal growth.
  • Coordinate trips, celebrations, and special events for young adults and their families to increase cultural pride and awareness.
  • Collaborate with community agencies, public officials, and organizations.
  • Participate in program team meetings to discuss accomplishments and program-related matters.
  • Represent the program to community providers and provide relevant information and training.
  • Document community activities, outings, and development involving families.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in human services or related field, preferably in social work.
  • Experience working with adolescents and families.
  • Bilingual skills in Spanish required.
  • Proficiency with MS Office and Google product suites.
  • Cultural competency in working with Hispanic adolescents and their immigrant families.
  • Proven ability to provide strong social service support and counseling.
  • Ability to apply analytical thinking to program matters regarding the Life is Precious Program.
  • Knowledge of program development and the ability to engage and motivate adolescents.
  • Responsible, organized, and efficient.
  • Demonstrated ability to perform in a team approach and accomplish multiple assignments.
  • Demonstrated skill to communicate effectively in writing, verbally, and to listen actively.
  • Related experience that demonstrates community-mindedness, patience, creativity, flexibility, compassion, and cultural sensitivity.
  • Ability to deal tactfully and diplomatically with other employees, officials, professional and technical groups, clients, and the general public.
  • Ability to plan and successfully implement and gather patient-related data for programs.
