About the position

The Case Manager role at the Lucas County Guardianship Services Board is a full-time position responsible for managing a caseload of adult guardianship clients. The position serves as a Guardian Designee, providing essential social services to vulnerable individuals in the community. The role requires a hybrid work model, with in-person meetings and on-call responsibilities, ensuring clients receive the necessary support and resources for their well-being.

Responsibilities

  • Manage an assigned caseload of adult guardianship ward clients, assisting with medical consent decisions, housing assistance, and budgetary plans.
  • Follow and understand LCGSB policies, procedures, and protocols.
  • Meet in-person with all assigned wards monthly, with more frequent contact as needed.
  • Conduct welfare checks on clients through home visits as necessary.
  • Maintain daily case plan notes and database entries, fulfilling reporting needs regarding ward clients.
  • Conduct client information searches regarding mental health and criminal history.
  • Assist the LCGSB SW/Case Management Team with case plans and social services.
  • Participate in Social Work Team discussions and events.
  • Conduct collaboration meetings with local agencies.
  • Work with multi-interdisciplinary teams to support clients.
  • Monitor vendor issues and communicate major issues to the SW Supervisor.
  • Provide necessary information to the Social Work Supervisor for Annual Guardian's Reports and Plans.
  • Participate in programs that enhance the Board's mission and professional development.
  • Meet weekly with the Social Work Team for case reviews and support.
  • Complete all mandatory training as determined by LCGSB Management.
  • Participate in the After Hours/On Call Phone Line rotation.
  • Protect clients through mandated reporting of Major Unusual Incidents.
  • Perform other duties as assigned.

Requirements

  • An Associate's degree in a Health and Human Services field (e.g., Criminal Justice, Nursing, Social Work, Psychology).
  • At least three years of experience in Case Management or Court Probation Case Work.
  • Successful completion of a criminal records background check and drug/alcohol screening.
  • A valid driver's license with driving privileges in Ohio.

Nice-to-haves

  • Experience with Microsoft Office applications.
  • Ability to work with a diverse population and handle sensitive inquiries.
