About the position

Apalachee Center, Inc. is dedicated to assisting individuals and families in North Florida in overcoming emotional, psychiatric, and substance abuse crises. The organization provides a wide range of behavioral health services across eight counties, including Franklin, Gadsden, Jefferson, Leon, Liberty, Madison, Taylor, and Wakulla. The RN position at the Central Receiving Facility (CRF) is crucial in ensuring that clients receive timely and appropriate care upon their arrival. The RN will be responsible for conducting pre-admission nursing screenings, triaging clients, and performing nursing assessments to gather essential information about each individual's medical history and current medications. This role requires effective communication with various stakeholders, including family members, nursing homes, and pharmacies, to ensure comprehensive care for clients. The RN will work closely with psychiatrists and nursing staff to review assessment findings and provide immediate medication as needed to address urgent medical or behavioral health symptoms. Crisis management is a key component of this role, which may involve initiating seclusion or Emergency Treatment Orders/Holds when necessary. Additionally, the RN will provide support to the team and co-supervise CRF Associates, drivers, and other team members in the absence of the CRF Team Leader. The RN's responsibilities also include ensuring compliance with medication management policies, conducting ongoing assessments of clients, and coordinating admissions to the facility to ensure a smooth transition for clients entering care. The position requires a commitment to maintaining high standards of care and documentation, as well as the ability to work collaboratively with a multidisciplinary team to support the needs of clients in crisis. The RN will play a vital role in the overall functioning of the CRF and contribute to the mission of Apalachee Center, Inc. to provide quality behavioral health services to the community.

Responsibilities

  • Completes pre-admission nursing assessments expeditiously to help team meet the 2 hour target for disposition to a receiving facility referral for admission.
  • Conducts admission Nursing Assessments and Medication Reconciliation for ACI admissions in accordance with Client Record Handbook and unit procedure specifications.
  • Documents according to JC standards and provides direct nursing care as needed in accordance with unit procedures and physician's orders.
  • Performs ongoing nursing supervision, assessment, and monitoring of clients while they are in the Access unit.
  • Notifies physician of changes in client's condition and documents ad hoc nursing care progress notes in accordance with unit procedures.
  • Verifies proper identification of client using two identifiers for each medication administration and clinical test procedure.
  • Maintains unit compliance with applicable medication management, storage, and narcotic control policies and procedures.
  • Coordinates and leads inter-shift report with nursing, Associates, Access Counselors, and other staff.
  • Assures compliance with established continuity of care and client safety change-of-shift procedures.
  • Coordinates all ACI admissions with the admitting unit nurses to ensure appropriate bed availability.

Requirements

  • Registered Nurse (RN) license in the state of Florida.
  • Experience in behavioral health or psychiatric nursing preferred.
  • Strong assessment and triage skills.
  • Ability to work in a fast-paced environment and manage crisis situations effectively.
  • Excellent communication and interpersonal skills.

Nice-to-haves

  • Experience with medication management and reconciliation.
  • Knowledge of community resources for behavioral health services.
  • Familiarity with electronic health record systems.
