About the position

As a Chaplain at Covenant Living Communities, you will be responsible for providing spiritual care services to patients, families, and caregivers within the CovenantCare Hospice program. This role is pivotal in ensuring that the spiritual needs of those in hospice care are met with compassion and understanding. You will function as a key contact with local clergy, facilitating a network of support and spiritual guidance for the community surrounding the campus. Your work will involve conducting spiritual assessments, developing individualized care plans, and providing direct spiritual care to those in need. In this position, you will actively participate in interdisciplinary team meetings, contributing to comprehensive assessments that address the spiritual needs of patients and their families. You will also be responsible for maintaining accurate records of spiritual care services, which will be used for quality assurance and program development. Additionally, you will assist families with bereavement support and may be called upon to provide funeral or memorial services as requested. Your role will require you to accept and perform other related duties as necessary, ensuring that all aspects of spiritual care are addressed in a timely and respectful manner.

Responsibilities

  • Assures spiritual assessment of patients, families and caregivers in the hospice program and appropriate services are rendered in a timely manner.
  • Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Provides direct spiritual care to patients, families and caregivers.
  • Serves as liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services utilization and related activities for quality assurance, program development and policies and procedures review and revision.
  • Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
  • Provides for funeral or memorial services for patients as requested.
  • Assists with bereavement support for families and caregivers.
  • Accepts and performs other related duties and responsibilities as required.

Requirements

  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
  • Experience working with death and dying, individuals/family/caregiver.
  • Comfort in interfaith setting.
  • Must demonstrate effective communication skills and articulate a passion for ministry characterized by servant leadership.
  • Demonstrate the ability to establish and maintain healthy and positive relationships with staff and residents.
  • Ability to accept different lifestyles, cultures, beliefs and values.
  • Ability to network with community clergy and congregations.
  • Knowledge and commitment to hospice philosophy of care.
  • Able to handle confidential information.
  • Must demonstrate good judgment, and possess problem solving skills and remain calm when faced with unusual circumstances.
