About the position

Gentiva is seeking a Client Success Manager for Value Based Care Partnerships to join our dedicated team. This role is pivotal in ensuring the successful service delivery and financial performance for one or more Illumia clients, particularly those with a multi-market presence. The Client Success Manager will report directly to the VP of Value Based Strategy and will be responsible for coordinating all communications, strategies, development, and growth opportunities for our clients. This position requires a collaborative approach, working closely with Illumia's market-based operational leadership and corporate functions to ensure high performance and a positive experience for our clients. In this role, you will function as the primary contact for our clients, ensuring that performance standards are met and that goals are achieved. You will collaborate with local Market Operations counterparts to drive success and align closely with Illumia's strategy and growth team on program enhancements. Your entrepreneurial mindset will be essential as you focus on creating, executing, and improving new functions that enhance client satisfaction and operational efficiency. The ideal candidate will possess strong presentation, written, and verbal communication skills, along with a minimum of 5 years' experience in account management or relevant fields. Experience interacting with mid-level and senior customer executives is crucial, as is a background in healthcare, whether as a provider, health plan, or vendor. Experience managing providers in value-based care is strongly preferred, as is experience in high growth environments and clinical operations management. A Bachelor's degree is required, with a graduate or healthcare degree being a plus.

Responsibilities
