The Client Success Manager at TheKey plays a crucial role in ensuring that clients receive consistent, high-quality care tailored to their individual needs. This position is dedicated to maintaining accurate and up-to-date client care plans through regular assessments and reports. The Client Success Manager monitors the delivery of care, analyzes client needs, and determines when additional or different care may be beneficial. Building strong relationships with clients and their support systems is essential, as the role encompasses addressing physical, emotional, and social well-being, while creating life-enriching opportunities that promote independence. In this role, the Client Success Manager is responsible for managing day-to-day client relationships and ensuring the delivery of quality care. This includes identifying a holistic approach to client care, recommending specific avenues to enhance the client experience, and utilizing strong interpersonal skills to build rapport with clients and their families. The position requires effective communication with both internal and external stakeholders, ensuring that the care team is well-informed and that care plans are thoroughly documented and updated regularly. The Client Success Manager also acts as a subject matter expert for quality control and assurance, conducting thorough initial assessments for new clients and following the Start of Care Process to guarantee a quality experience. Regular in-person re-assessments and quality assurance visits are conducted to evaluate home safety, daily routines, and client preferences. The role involves proactive assessment of client needs, effective communication with the interdisciplinary team, and managing caregiver introductions and training as care needs evolve. Additionally, the Client Success Manager addresses escalations and complaints, reviews client rates, ensures compliance with state regulations, and seeks cross-referral opportunities to enhance business growth.

Responsibilities

  • Managing the day-to-day client relationship and delivery of quality care
  • Identifying and focusing on a holistic approach to client care with a focus on quality of life
  • Utilizing interpersonal relationships and communication skills to build rapport with clients
  • Ensuring that TheKey care team is up-to-date with all relevant information and care plans
  • Collaboration and communication with internal and external customers
  • Consulting with clients regarding their specific needs and preferences
  • Acting as subject matter expert for quality control and quality assurance matters
  • Conducting thorough initial assessments for new clients
  • Following the Start of Care Process to ensure a quality experience
  • Conducting in-person client re-assessments and quality assurance visits
  • Proactively assessing and reassessing client needs and referring to additional services
  • Communicating effectively with the interdisciplinary team
  • Managing Caregiver introductions to clients and families
  • Managing escalations and/or complaints from clients and caregivers
  • Reviewing and adjusting client rates as care progresses
  • Ensuring compliance with all state regulations and company guidelines
  • Recommending and/or implementing policies and procedures for clients
  • Looking for cross-referral opportunities to increase business
  • Supporting on-call and after-hours support as needed

Requirements

  • Bachelor's Degree in Human or Health Services or related profession from an accredited university preferred
  • 3 years' experience in health care, elder care, social work or related industry preferred
  • Licensure as an RN/LVN/MFT may be substituted for some portion of previous experience
  • Excellent customer service and conflict resolution skills
  • Computer proficiency and ability to document accurate and timely notes in systems related to client visits
  • Current driver's license and proof of insurance
