About The Position

The Clinical Navigator at PAM Health is a pivotal role that serves as the face of the hospital, engaging with patients during their rehabilitation journey. This position focuses on cultivating referral relationships, managing patient assessments, and collaborating with various stakeholders to facilitate successful admissions and enhance patient satisfaction. The Clinical Navigator plays a crucial role in promoting the hospital's services and ensuring that patients receive the support they need to achieve their rehabilitation goals.

Requirements

• Degree from an accredited college or university in a health field, preferably nursing.
• Current licensure in nursing or respiratory therapy is required.
• Minimum of two years' experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Nice To Haves

• Prior marketing experience in rehabilitation or LTACH settings.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data from assigned territory.
• Collaborates with business development team members, including the Admissions Manager and admission staff.
• Facilitates referral conversion by working with nursing and other internal and external staff.
• Provides feedback and recommendations for program development and quality improvement initiatives.
• Develops and maintains excellent relationships with prospective patients, family members, physicians, and other stakeholders.
