About the position

The ED Clinical Pharmacist ensures safe and effective medication use by reviewing orders, preparing medications, advising on drug interactions, and participating in patient care decisions, especially during emergencies. Key responsibilities of an ED pharmacist include medication order review, medication preparation, patient safety, collaboration, medication reconciliation, education, quality improvement, emergency response, and discharge counseling.

Responsibilities

  • Reviewing physician orders for accuracy, appropriateness, and potential interactions, ensuring safe and effective drug therapy.
  • Preparing medications for immediate use, ensuring proper dosage and administration routes, especially during emergencies.
  • Identifying and preventing medication errors, adverse drug events, and ensuring appropriate medication use in the high-risk ED environment.
  • Collaborating with physicians, nurses, and other healthcare providers to optimize patient care, particularly during resuscitation and critical situations.
  • Taking medication histories and reconciling medications upon admission and discharge to ensure continuity of care.
  • Providing education to medical and nursing staff on medication-related topics and best practices.
  • Participating in quality improvement initiatives to enhance medication safety and optimize drug use within the ED.
  • Responding to medical emergencies, assisting with resuscitation efforts, and ensuring timely access to necessary medications.
  • Providing medication counseling to patients upon discharge, ensuring they understand their medications and how to take them safely.

Requirements

  • Bachelor's Degree in Pharmacy
  • 2 - 3 years of relevant experience
  • Doctor of Pharmacy Degree (Pharm.D.) preferred
  • PGY1 and PGY2 Residency in ED Preferred
  • BCPS

  • Some travel, to follow-up with Corporate, Financial, GPO, Compliance and Pyxis issues.
