About The Position

The Community Health Worker (CHW) will engage directly with individuals who use substances, facilitating their access to substance use, mental health, and medical services. This role is crucial for supporting Texas residents living with substance use disorders (SUD) and requires the CHW to obtain state certification within six months of employment. The position operates independently with limited supervision, reporting major activities through periodic meetings.

Requirements

• High School Diploma or GED required.
• At least 2 years of experience in substance abuse and/or mental health fields.
• Minimum of two years of experience in substance use outreach, intervention, or treatment.
• At least one year of experience working with prison populations, individuals with housing instability, or those with SUDs, HIV/STDs, and/or behavioral health issues.

Responsibilities

• Work directly with individuals who use substances to facilitate linkages to services.
• Assist in the retention of individuals in substance use, mental health, and medical services.
• Conduct outreach and intervention for substance use disorders.
• Provide support to individuals experiencing housing instability or behavioral health issues.
