About the position

The Community Support Specialist Crisis Integration is responsible for conducting outreach and coordinating care with existing Compass Health clients who utilize the Crisis Access Points. This position serves the Western region of Compass Health Network.

Responsibilities

  • Coordinates with CPRC, outpatient and crisis services for all active CPRC or YCPRC clients that are referred to ERE/YERE or seen at one of the Crisis Access Points (CAP) but have existing services.
  • Meets with CPRC clients at the Crisis Access Points, as needed to offer additional support for clients needing a higher level of care.
  • Coordinates with all CHN programs with clients in crisis; assisting with basic needs, attending appointments & obtaining resources.
  • Performs outreach and follow up with clients as required in person.
  • Transports CPRC clients to the CAP as needed.
  • Ensures all ERE/CAP CPRC monthly chart reviews are completed thoroughly and in a timely manner.
  • Attends ERE/CAP/integrated meetings.
  • Works with the client to determine barriers, if any, to participating in their current programs.
  • Completes hospital follow-up documentation with CPRC/YCPRC clients as needed.
  • Reconnects and re-engages CPRC clients with their providers.
  • Reviews the regional CAP and ERE spreadsheets and referrals daily for CPRC referrals.
  • Completes CAP and ERE data for all CPRC clients referred.
  • Documents all coordination of care/services within 24 hours.
  • Driving is an essential function of this position.

Requirements

  • High School/GED required.
  • Associate's/Bachelor's degree in a human service-related field preferred.
  • At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained.
  • At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.
