About the position

The Community Support Specialist CSTAR/ICTS provides integrated, holistic, community-based support services for the CSTAR and/or ICTS teams. This role is essential in delivering quality and compassionate care to the communities served, focusing on supporting clients' movement toward recovery and resiliency goals through individualized and group education services. The position involves assisting clients in identifying their strengths and needs, developing Wellness Plans, and providing holistic support across various domains of wellness. The specialist will also maintain contact with hospitalized clients, assist in discharge planning, and provide education in daily living skills, symptom management, and personal support systems.

Responsibilities

  • Provide individualized and group education services based on clients' strengths, needs, abilities, and preferences.
  • Assist clients in developing Wellness Plans and support them in achieving their goals.
  • Aid clients in accessing public services such as financial, housing, and emergency assistance.
  • Maintain contact with hospitalized clients and participate in discharge planning.
  • Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
  • Assist clients in developing personal support systems.
  • Follow up with clients regarding appointments and necessary documentation.
  • Re-engage clients who have unexcused absences from the CSTAR program.
  • Complete documentation and paperwork within required timeframes.
  • Collect and process urine drug screens.
  • Communicate with integrated health team members regarding client wellness.
  • Make referrals to additional services/providers as needed.
  • Provide information about RCORP grant to community partners.
  • Assist clients with transportation as clinically indicated.

Requirements

  • High School/GED required.
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred.
  • Must qualify based on overall CSS requirements in at least one of the specified options.
  • Valid driver's license and agency established minimum automobile coverage required.
  • Qualifying experience must include delivery of service to individuals with mental illness, substance use disorders, or developmental disabilities.
