About the position

The Community Support Specialist CSTAR/ICTS provides integrated, holistic, community-based support services for the CSTAR and/or ICTS teams. This role is essential in delivering quality and compassionate care to the communities served, focusing on supporting clients' movement toward recovery and resiliency goals through individualized and group education services. The position involves assisting clients in identifying their strengths and needs, developing Wellness Plans, and providing holistic support across various domains of wellness. The specialist will also maintain contact with clients, assist in accessing public services, and provide education in daily living skills and symptom management.

Responsibilities

  • Provide individualized and group education services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Assist clients in identifying strengths, needs, abilities, and preferences regarding substance abuse/mental illness.
  • Support clients in completing goals identified on Wellness Plans and review them as needed.
  • Aid clients in accessing public services, including financial, housing, and emergency assistance.
  • Maintain contact with hospitalized clients and participate in discharge planning.
  • Provide education and skill-building in areas such as daily living skills, employment/education, and budgeting.
  • Assist clients in developing personal support systems.
  • Provide individual assistance to clients in accessing needed services.
  • Follow up with clients regarding appointments and completion of forms.
  • Contact consumers with unexcused absences to re-engage them.
  • Complete documentation and paperwork within the required timeframe.
  • Collect and process urine drug screens.
  • Communicate actively with other members of the integrated health team regarding client wellness.
  • Make referrals to additional services/providers as needed.
  • Provide information about RCORP grant to community partners.
  • Carry program phone on a rotational basis and answer during business hours.
  • Seek culturally competent services to meet the unique needs of clients.
  • Assist clients with transportation as clinically indicated.

Requirements

  • High School/GED required.
  • Must qualify for position based on overall CSS requirements.
  • Qualified addiction professional or Bachelor's degree in a human services field.
  • Any four-year degree or combination of higher education and qualifying experience.
  • Four years of qualifying experience in delivering service to individuals with mental illness, substance use disorders, or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves

  • Associate's/Bachelor's/Master's degree in a human service-related field preferred.
