About the position

The Community Support Specialist for the Emergency Room Enhancement program is responsible for engaging consumers into services, assisting them in accessing needed services and resources to achieve and maintain health and recovery. This role is crucial in providing quality and compassionate care in the communities served by Compass Health Network.

Responsibilities

  • Provide individual assistance to consumers in accessing needed mental health and medical services
  • Coordinates care with internal and external providers regarding client care
  • Provide individuals assistance to consumers in accessing a variety of public services including financial and housing, including assistance on an emergency basis and directly helping to meet needs for food, shelter and clothing
  • Assist consumers to access and utilize a variety of community agencies to acquire ongoing social, educational, vocational and health and housing resources
  • Training, coaching and supporting in daily living skills, including housekeeping, cooking, personal grooming, accessing transportation, keeping a budget, paying bills and maintaining an independent residence
  • Follow up with consumers regarding appointments, completion of forms, returning forms or receipts and other similar activities
  • Attempts to outreach all new referrals within 24 hours
  • Assist client with transportation as clinically indicated
  • Attends weekly supervisions with the ERE supervisor
  • Driving is an essential function of this position
  • Other duties as assigned

Requirements

  • High School/GED required
  • Associate's/Bachelor's/Master's degree in a human service-related field preferred
  • Must qualify for position based on overall CSS requirements in at least one of the options listed: Qualified addiction professional; Bachelor's degree in a human services field from a college or university included in the U.S. Department of Education's database of accredited schools; Any four-year degree or combination of higher education and qualifying experience; Four years of qualifying experience; or Associate of Applied Science in Behavioral Health Support degree as designated by the department.
  • Qualifying experience must include delivery of service to individuals with mental illness, substance use disorders, or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required
