About the position

The Community Support Specialist provides integrated, holistic, community support services engaging both the child and parent/caregiver. This role is crucial in supporting clients and their families in achieving improved functioning and treatment goals through a collaborative and strengths-based approach. The position involves working closely with families, providing educational information, and developing crisis intervention plans as needed. Services are provided in the home based on clinical need, including evenings and weekends.

Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Engage parents in treatment to improve clinical outcomes for the child.
  • Utilize a holistic, person-centered, strengths-based perspective in service provision.
  • Support client/family in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing and arranging services and resources to achieve their goals.
  • Maintain contact with clients during hospitalization and participate in discharge planning.
  • Provide educational information and skill building to clients and families.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist clients and families in developing natural support systems.
  • Participate in coordination with interagency team members to advocate for mental health needs.
  • Complete documentation collaboratively with clients and families within required timeframes.
  • Meet service provision expectations per client needs and agency guidelines.

Requirements

  • High School diploma or GED required.
  • Associates or Bachelor's degree in a human service-related field preferred.
  • At least four years of experience or a combination of higher education and experience if only a High School/GED is obtained.
  • At least two years of experience or a combination of higher education and experience if an Associate's degree is obtained.
  • At least one year of experience required if a non-human service Bachelor's degree is obtained.
  • Experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves

  • Experience in community support services or mental health settings.
  • Knowledge of the System of Care philosophy and Eight Dimensions of Wellness.
