About the position

The Community Support Specialist provides integrated, holistic, community support services engaging both the child and parent/caregiver. This role is crucial in supporting clients and their families in achieving treatment goals and improving overall functioning. The position involves working closely with families, providing educational information, and developing crisis intervention plans as needed. Services are provided in the home based on clinical need, including evenings and weekends.

Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Engage parents in treatment to improve clinical outcomes for the child.
  • Provide services using a holistic, person-centered, strengths-based perspective, incorporating coaching surrounding the Eight Dimensions of Wellness.
  • Support client/family in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing and arranging services and resources to achieve their goals.
  • Maintain contact with clients during hospitalization and participate in discharge planning.
  • Provide educational information and skill building regarding daily living skills, coping skills, and symptom management.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist client/family in developing natural support systems.
  • Participate in coordination with interagency team members to advocate for the mental health needs of the child/family.
  • Complete documentation collaboratively with client and family within required timeframes.
  • Meet service provision expectations per client needs and agency guidelines.

Requirements

  • High School diploma or GED required.
  • Associates or Bachelor's degree in a human service-related field preferred.
  • At least four years of experience in a relevant field if no degree is obtained.
  • At least two years of experience if an Associate's degree is the highest education obtained.
  • At least one year of experience if a non-human service Bachelor's degree is the highest education obtained.
  • Experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves

  • Experience working in a community health setting.
  • Knowledge of the System of Care philosophy.
  • Familiarity with the Eight Dimensions of Wellness.
