About the position

The Community Support Specialist provides integrated, holistic, community support services engaging both the child and parent/caregiver. This role is essential in supporting clients and their families in achieving treatment goals and improving overall functioning. The position involves working collaboratively with clients and families to develop and implement wellness plans, provide educational information, and assist in accessing necessary resources. The Community Support Specialist will also maintain communication with clients during hospitalizations and participate in discharge planning, ensuring continuity of care. This is a full-time, benefit-eligible position with a focus on providing services in the home and community settings, including evenings and weekends as needed.

Responsibilities

  • Provide individualized services based on strengths, needs, abilities, and preferences identified collaboratively with client/family.
  • Engage parents in treatment to improve clinical outcomes for the child.
  • Utilize a holistic, person-centered, strengths-based perspective in service provision.
  • Support client/family in completing goals identified on Wellness Plans and reassess needs as necessary.
  • Assist families in accessing and arranging services and resources to achieve their goals.
  • Maintain contact with clients during hospitalizations and participate in discharge planning.
  • Provide educational information and skill-building to clients and families regarding daily living skills, coping skills, and symptom management.
  • Develop and implement crisis intervention plans and provide crisis intervention services as needed.
  • Assist client/family in developing natural support systems.
  • Participate in coordination with interagency team members to advocate for the mental health needs of the child/family.
  • Complete documentation collaboratively with client and family within required timeframes.
  • Meet service provision expectations per client needs and agency guidelines.

Requirements

  • High School diploma or GED required.
  • At least four (4) years of experience in a relevant field if no degree is obtained.
  • At least two (2) years of experience required if an Associate's degree is the highest education obtained.
  • At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained.
  • Experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities.
  • A valid driver's license and agency established minimum automobile coverage required.

Nice-to-haves

  • Associates or Bachelor's degree in a human service-related field preferred.
