About the position

The Community Support Specialist plays a vital role in helping individuals navigate the challenges of daily life, empowering them to thrive within their communities. By employing evidence-based strategies and fostering collaboration with clients, families, and community partners, the Community Support Specialist supports individuals in achieving long-term stability, resilience, and independence. This role involves creating personalized plans, providing targeted interventions, and connecting clients with local resources to address their unique needs. Responsibilities

  • Provide community-based mental health services to individuals across the lifespan, including assessments, treatment planning, and intervention implementation.
  • Collaborate with clients, families, and multidisciplinary teams to create and deliver individualized care plans that emphasize empowerment, resilience, and personal growth.
  • Utilize evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, Positive Reinforcement Techniques, and skills rooted in CBT and DBT frameworks.
  • Offer behavioral interventions, including cognitive processing, group dynamics, and operant conditioning, tailored to client needs.
  • Coordinate care with external providers such as primary care physicians, psychiatrists, educators, and local organizations to ensure a holistic approach to client support.
  • Facilitate and lead group sessions related to psycho-social rehabilitation (PSR), based on client treatment goals.
  • Maintain accurate and timely documentation of client interactions, progress, and treatment outcomes, adhering to organizational policies and legal requirements.
  • Participate in professional development opportunities, staying current on best practices in mental health care and evidence-based techniques.
  • Actively contribute to quality improvement initiatives through data collection, reporting, and collaborative efforts.
  • Develop and maintain strong relationships with community stakeholders, including families, educators, and other key influencers in clients' support systems.
  • Respond effectively to client crises, ensuring appropriate resources and interventions are in place.

Requirements

  • Bachelor's degree in Psychology, Social Work, or a related human services field; or
  • Bachelor's degree in an unrelated field with two years of relevant experience; or
  • Any combination of four years of higher education and related experience; or
  • Associate of Applied Science in Behavioral Health Support; or
  • Four years of qualifying experience delivering services to individuals with mental health disorders, substance use disorders, or developmental disabilities.
