The Social Services Director is responsible for planning and administering social service programs within the nursing home environment. This role involves supervising nursing home social workers and assisting in the development of policies regarding community planning for health and welfare services. The Social Services Director plays a crucial role in ensuring that residents receive comprehensive social services that address their physical, emotional, and social needs. This includes planning and directing a social services program that provides counseling and support services for residents and their families, as well as conducting evaluations of each resident's social needs and formulating care plans accordingly. In addition to direct service provision, the Social Services Director consults with the Administrator and other department heads on interdisciplinary issues, ensuring that all aspects of resident care are coordinated effectively. The position requires maintaining written records of social service consultations and services provided, as well as collecting pertinent social data upon resident admission. The Director also acts as a liaison between residents, families, outside agencies, and the facility's administration, advocating for residents' rights and ensuring they receive the highest quality of care in a compassionate environment. The role includes responsibilities related to discharge planning, where the Director develops organized discharge plans for residents, and ensures compliance with legal, safety, health, fire, and sanitation codes. The Social Services Director must also perform administrative tasks, such as completing necessary forms and reports, and may assume responsibilities of the Admissions department in the absence of the Admissions/Marketing Director. Overall, this position is vital in fostering a supportive and caring atmosphere for residents, while adhering to all facility policies and procedures.
