About the position

The Social Services Director is responsible for planning and administering social service programs within the nursing home environment. This role involves supervising nursing home social workers and assisting in the development of policies regarding community planning for health and welfare services. The Social Services Director plays a crucial role in ensuring that residents receive comprehensive social services that address their physical, emotional, and social needs. This includes planning and directing a social services program that provides counseling and support services for residents and their families, as well as conducting evaluations of each resident's social needs and formulating care plans accordingly. In addition to direct service provision, the Social Services Director consults with the Administrator and other department heads on interdisciplinary issues, ensuring that all aspects of resident care are coordinated effectively. The position requires maintaining written records of social service consultations and services provided, as well as collecting pertinent social data upon resident admission. The Director also acts as a liaison between residents, families, outside agencies, and the facility's administration, advocating for residents' rights and ensuring they receive the highest quality of care in a compassionate environment. The role includes responsibilities related to discharge planning, where the Director develops organized discharge plans for residents, and ensures compliance with legal, safety, health, fire, and sanitation codes. The Social Services Director must also perform administrative tasks, such as completing necessary forms and reports, and may assume responsibilities of the Admissions department in the absence of the Admissions/Marketing Director. Overall, this position is vital in fostering a supportive and caring atmosphere for residents, while adhering to all facility policies and procedures.

Responsibilities

  • Plans and directs a comprehensive social services program which provides for counseling and other support services for residents and families.
  • Consults with the Administrator and other department heads regarding interdisciplinary issues.
  • Works directly with residents and families experiencing personal and environmental difficulties or concerns related to the resident's physical or emotional condition.
  • Assures that medically related social services are provided to maintain or improve each resident's ability to control everyday physical needs and mental and psychosocial needs.
  • Maintains a written record of the frequency and nature of the social service consultation and services provided or obtained.
  • Performs an evaluation of each resident's social needs and formulates a care plan recorded in the resident's medical record.
  • Collects pertinent social data upon admission of each resident to be placed in the medical record, including information about personal and family problems related to the resident's illness and care.
  • Assists each resident with adjustment to the social and emotional aspects of placement, including clearly defined Social Service goals/interventions in the care plan.
  • Coordinates the interdisciplinary care plan team, schedules conferences in a timely manner, and invites residents/family members as appropriate.
  • Acts as a liaison between residents, families, outside agencies, and the facility Administrator to ensure that the resident's rights are maintained.
  • Assumes the roles and responsibilities of the Admissions department in the absence of the Admissions/Marketing Director.
  • Functions in a management capacity and adheres to all policies/procedures of the facility as a representative of the Windsor Gardens Care Center's administration.
  • Performs administrative requirements, such as completing necessary forms and reports, under the direction of the Administrator.
  • Coordinates discharge planning, including the development of an organized discharge plan for all residents.
  • Ensures the safety of all residents to minimize the potential for fire and accidents, and ensures compliance with legal, safety, health, fire, and sanitation codes.
  • Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual's needs and rights.
  • Performs other duties as requested.

Requirements

  • Bachelor's degree in Social Work or a bachelor's degree in a human services field including Sociology, Special Education, Rehabilitation Counseling, and Psychology.
  • Master's degree preferred.
  • Any certifications required by New Jersey State Regulations are also necessary for this position.
  • 1 year of experience in a senior level role related to social services.
  • Excellent interpersonal skills.
  • Highly developed written and verbal communication skills.
  • Knowledge of all governmental regulations relating to the Social Service Department.
  • Problem-solving skills.
  • Provide annual verification of a negative TB skin test.
