Responsibilities

  • Provides personal care to PACE participants both in the PACE center and out in the community, when necessary.
  • Exercises independent judgement; reports any changes in participant status to the IDT in morning meeting or as needed.
  • Provides day-to-day supervision to direct reports; coordinates schedules for all direct reports at both the PACE center and out in the community.
  • Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines.
  • Able to navigate EMR and software used for authorizations of service needs in the home; Able to navigate and place orders for DME with vendors.
  • Assists with needs of participants at the PACE center and out in the community as needed under the supervision of the SE Supervisor, Clinic or designee.
  • Contributes to the development of a care plan for participants through interaction with other members of the Interdisciplinary Team.
  • Collaborates with members of the Interdisciplinary team to assure appropriateness and continuity of care.
  • Carries out non-skilled treatments including, but not limited to transfers, toileting, bathing, dressing at the PACE Center and periodically in the community as needed.
  • Assists the SE Supervisor, Clinic or designee as well as Life Enrichment staff with individual and group programs by helping to plan individual treatment programs, increasing participants' motivation to participate, assisting participants to participate when needed, and assisting with evaluation of program effectiveness.
  • Maintains a clean and safe working environment in the PACE center.
  • Oversees direct reports' daily tasks and assignments; regularly meets with staff.
  • Uses safe techniques in all interactions with participants at the PACE Center.
  • Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions.
  • Provides feedback to direct reports on their performance on a regular basis.
  • Keeps SE Supervisor, Clinic or designee informed of any health aide issues or challenges.
  • Attends IDT meetings keeping track of initial services; and any changes to service needs through the review period.
  • Inputs and coordinates changes to authorization needs with vendors.
  • Coordinates services required in the home (home delivered meals; durable medical equipment; aide service requirements).
  • Initiates and makes changes/updates to product and service orders with vendors.
  • Initiates and manages required authorizations for service needs using EMR and software required to generate documents.
  • Performs all duties in accordance with FH Care policies and procedures.
  • Responsible to ensure Care Plans are signed by Health Aide on each assigned day.

Requirements

  • High school graduate or equivalent.
  • Certification as a Home Health Aide or Nursing Assistant desirable.
  • CPR and First Aid Certification or willingness to be certified with 60 days of employment is required.
  • 2-4 years of experience working with a frail or elderly population.
  • At least one year in a lead or supervisory capacity.
  • Must complete a standardized set of competencies for personal care assistance and service coordination.
