About The Position

The Home Health LPN at HCA Florida Healthcare at Home is responsible for administering quality nursing care to patients in their homes. This role involves working under the supervision of a Registered Nurse and following physician orders to ensure effective patient care. The LPN will assess patient needs, document care provided, and communicate with healthcare team members to maintain high standards of care.

Requirements

• Graduate of an accredited school of professional nursing.
• Minimum of one or more years of home health, public nursing, or acute hospital nursing experience.
• Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS is strongly preferred.
• Reliable transportation and proof of valid automobile liability insurance.
• Must have a valid driver's license.
• Current BCLS Certification prior to providing patient care.

Responsibilities

• Administers skilled nursing care to patients in the home under the direction of a Registered Nurse.
• Verifies the Plan of Care prior to each visit and provides care according to physician's orders and established standards.
• Performs ongoing assessments during each visit and documents findings in the patient medical record.
• Communicates significant findings and changes in the patient's condition to the supervising RN and documents all interventions.
• Assesses patient/caregiver's knowledge of disease processes and provides education as needed.
