About the position

The Home Health Nurse Assessor role at Astyra Corporation is designed for Registered Nurses (RNs) who are passionate about making a difference in the lives of North Carolinians. This position involves conducting assessments for Medicaid beneficiaries to determine eligibility for personal care services, ensuring that individuals receive the necessary support for daily living activities. The role offers flexibility in scheduling, allowing nurses to work from home and travel within their designated regions to perform assessments. This position is integral to the North Carolina Medicaid Linking Individuals & Families for Long-Term Services and Supports (NC LIFTSS) project, which aims to streamline access to long-term services and supports.

Responsibilities

  • Conduct assessments to determine eligibility for Medicaid-funded personal care services using state-approved standardized assessment tools.
  • Ensure that personal care services are provided based on the beneficiary's unmet needs, considering their medical condition and functional abilities.
  • Maintain the privacy and dignity of individuals during assessments.
  • Conduct various types of assessments (new, expedited, annual, etc.) within established timeframes.
  • Interview family members and informal caregivers during assessments.
  • Guide beneficiaries in selecting personal care service providers.
  • Review service plans as needed and submit completed assessments using state-approved interfaces.
  • Participate in mediation and appeal processes for beneficiaries.
  • Respond to state inquiries regarding assessments conducted.
  • Attend staff meetings and conduct case consultations or peer reviews as assigned.
  • Develop level of care recommendations based on clinical evaluations.
  • Participate in training for personal care services stakeholders as needed.

Requirements

  • Registered Nurse licensed by the state of North Carolina.
  • Minimum of two years' experience in a home care setting.
  • Proficiency in Microsoft Excel, Word, and Outlook, and ability to use web-based software for work.
  • Experience with community-based individuals needing personal assistance with ADL and IADL tasks is highly preferred.
  • Experience conducting personal care services assessments is highly preferred.
  • Familiarity with the OASIS tool (or similar) is highly preferred.
  • Knowledge of North Carolina Medicaid Clinical Coverage Policy and PCS Program Provider Manual is highly desired.
  • Understanding of standards of practice related to Medicaid-funded Personal Care Services and home and community-based services is highly desired.

Nice-to-haves

  • Experience with OASIS tool or similar assessment tools.
  • Knowledge of North Carolina Medicaid Clinical Coverage Policy and PCS Program Provider Manual.
