About The Position

The Home Health RN position at HCA Virginia Healthcare at Home involves providing coordinated skilled nursing care to patients of all age groups in their homes. The role emphasizes collaboration with an interdisciplinary team to achieve patient goals and maintain high-quality care, under the supervision of a Clinical Manager.

Requirements

• Basic Cardiac Life Support must be obtained within 30 days of employment start date.
• Registered Nurse license required.
• Valid Driver's License required.
• Associate Degree or Registered Nurse Diploma required.

Nice To Haves

• Experience in home health care is preferred but not mandatory.

Responsibilities

• Assess home care patients identifying physical, psychosocial and environmental needs.
• Complete OASIS, assessment and visit paperwork according to agency policy.
• Ensure clinical notes accurately indicate communication and coordination of services with the physician and other team members.
• Communicate significant findings, problems, and changes to the Clinical Manager and physician, documenting all findings and interventions.
• Supervise and provide clinical direction to home health aides and LPNs/LVNs.
• Participate in on-call rotation and emergency calls according to agency policy.
