About The Position

The Hospice Nurse Practitioner is responsible for overseeing the clinical care and treatment of patients and families in a hospice setting. This role involves collaborating with the Interdisciplinary Group (IDG) to ensure quality palliative care, consulting with attending physicians, and providing education to patients and families regarding end-of-life care. The position requires effective communication and clinical assessment skills to address the needs of patients and their families.

Requirements

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse and as an Advanced Practice Nurse (APRN).
• Current Driver's License.
• Two years experience as an Advanced Practice Nurse in a clinical care setting, preferably in hospice or home health.
• Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.
• Good interpersonal skills.

Nice To Haves

• Experience in hospice care (1 year preferred).
• Willingness to travel (75% preferred).

Responsibilities

• Collaborates with the IDG to ensure appropriate evaluation and palliative treatment of terminal diseases.
• Consults with and assists in the IDG's development of patient/family care plans.
• Educates patients and families regarding disease processes, self-care techniques, and end-of-life care.
• Provides training to other staff as needed.
• Performs and documents visits to patients/families based on care plans and needs.
• Interacts with attending and consulting physicians regarding palliative care.
• Provides education to other IDG members on clinical issues related to palliation.
• Completes face-to-face encounters with patients as assigned.
• Communicates effectively and timely with physicians and personnel regarding patient needs and care outcomes.
• Maintains education and knowledge in pharmacology and palliative medicine.
