About the position

The Hospice RN position at Journey Hospice involves overseeing a primary caseload of hospice patients, providing skilled nursing care in compliance with relevant regulations and standards. The role emphasizes collaboration with the interdisciplinary team to develop and implement comprehensive care plans, ensuring that the needs of patients and their families are met effectively. The position also includes teaching and supervising hospice aides and engaging with families to support their caregiving roles.

Responsibilities

  • Oversee a primary caseload of hospice patients and provide skilled professional nursing care.
  • Conduct initial and comprehensive nursing assessments of each patient.
  • Reassess nursing needs and collaborate to develop the Hospice Interdisciplinary Team (IDT) plan of care.
  • Implement and revise the plan of care based on the patient's nursing needs.
  • Teach, supervise, and counsel patients, family members, and staff regarding nursing care and related problems.
  • Work closely with other IDT members to ensure all patient and family care needs are met.
  • Provide supervision to Hospice Aides/Homemakers.

Requirements

  • Graduate of an accredited registered nursing program.
  • NJ RN Licensure by the NJ State Board of Nursing in good standing.
  • Minimum of one (1) year general nursing experience preferred, with current hospice, home health, medical, surgical, or critical care experience.
  • Valid driver's license and reliable transportation that is insured in accordance with NJ state requirements.

Nice-to-haves

  • Bachelor's degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • Experience with pain and symptom management interventions preferred.
  • Understanding of Hospice philosophy and principles of death/dying.
