About The Position

As a Social Worker at Transitions Care, you will provide essential psychosocial services to terminally ill patients and their families, focusing on their emotional, spiritual, and practical needs. This role involves conducting assessments, implementing care plans, and facilitating access to community resources, all while ensuring compassionate support throughout the hospice experience.

Requirements

• Bachelor's degree in Social Work; Master's degree preferred.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years of social service experience in a healthcare setting or social services agency.
• Knowledge of community resources and eligibility requirements for state/federal financial programs.
• Strong understanding of hospice philosophy and principles of death/dying.
• Ability to work in an interdisciplinary setting with excellent observation and communication skills.
• Self-directed with the ability to work with minimal supervision.

Responsibilities

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments of patients and families.
• Provide ongoing support to patients, families, and caregivers.
• Facilitate access to community resources, including financial assistance.
• Assist families with referrals to community resources as needed.
• Help families with Advanced Directives and placement of patients in care facilities when appropriate.
• Maintain accurate patient records and submit relevant notes regarding care provided.
• Participate in team meetings and contribute to patient care planning.
• Provide continuity of care in collaboration with other team members.
• Participate in evening/weekend on-call services as required.
• Maintain clinical records and statistics for evaluation and reporting.
