The Housing Coordinator plays a vital role within the Assertive Community Treatment (ACT) team, focusing on providing essential housing assistance to clients and managing the Supported Apartment Program (SAP). This position is designed for individuals who are passionate about making a difference in the lives of those with serious mental illness (SMI) diagnoses. The Housing Coordinator will act as a Housing Specialist, facilitating access to appropriate housing supports and services tailored to the needs of clients receiving ACT and/or SAP housing services. This role emphasizes a holistic approach to client care, addressing social determinants that impact housing stability and overall wellbeing. In this position, the Housing Coordinator will engage with various community partners, including the Department for Community Based Services (DCBS), Lexington Housing Authority, and mental health advocacy groups, to create a collaborative environment that fosters positive relationships and effective service delivery. The Coordinator will be responsible for monitoring client engagement in clinical services, reporting key metrics to the Regional Director of Specialty and Outreach Services, and leading innovative strategies to enhance client participation in the program. The role requires strong communication skills, both oral and written, and the ability to work independently while managing multiple responsibilities. The Housing Coordinator will also oversee the financial aspects of the SAP program, ensuring sustainability and exploring innovative approaches to utilize financial resources effectively.
