About the position

The Housing Coordinator plays a vital role within the Assertive Community Treatment (ACT) team, focusing on providing essential housing assistance to clients and managing the Supported Apartment Program (SAP). This position is designed for individuals who are passionate about making a difference in the lives of those with serious mental illness (SMI) diagnoses. The Housing Coordinator will act as a Housing Specialist, facilitating access to appropriate housing supports and services tailored to the needs of clients receiving ACT and/or SAP housing services. This role emphasizes a holistic approach to client care, addressing social determinants that impact housing stability and overall wellbeing. In this position, the Housing Coordinator will engage with various community partners, including the Department for Community Based Services (DCBS), Lexington Housing Authority, and mental health advocacy groups, to create a collaborative environment that fosters positive relationships and effective service delivery. The Coordinator will be responsible for monitoring client engagement in clinical services, reporting key metrics to the Regional Director of Specialty and Outreach Services, and leading innovative strategies to enhance client participation in the program. The role requires strong communication skills, both oral and written, and the ability to work independently while managing multiple responsibilities. The Housing Coordinator will also oversee the financial aspects of the SAP program, ensuring sustainability and exploring innovative approaches to utilize financial resources effectively. This includes coordinating with ACT and other New Vista programs to maximize efficiency in client care and support. The position requires regular interaction with clients to assess their needs, assist with achieving mental wellbeing, and promote independent living skills. Overall, the Housing Coordinator is a key player in the integrated care system, dedicated to improving housing access and stability for individuals with SMI diagnoses.

Responsibilities

  • Provide housing assistance for clients and manage the Supported Apartment Program (SAP).
  • Facilitate access to appropriate housing supports and services for clients with serious mental illness (SMI).
  • Create a collaborative environment with key partners including DCBS, Lexington Housing Authority, and mental health advocacy groups.
  • Act as an ambassador of New Vista, connecting community partners and clients to services within the organization.
  • Serve as an expert collaborator on housing resources and community partnerships for individuals with SMI.
  • Prioritize client access to care according to need, ensuring prompt intakes and timely follow-up care.
  • Communicate and coordinate with multiple community partners, acting as the face of New Vista.
  • Monitor client engagement in clinical services and report key metrics to the Regional Director of Specialty and Outreach Services.
  • Oversee SAP program finances and address sustainability issues.
  • Problem-solve independently for client functioning and resource needs.
  • Meet with clinical teams, doctors, family members, and guardians to discuss client functioning.
  • Attend staff meetings and supervisions to assess client needs and opportunities.
  • Support medication acquisition and treatment engagement for clients.
  • Interact regularly with clients to determine needs and assist with achieving independent living goals.
  • Assist clients with finances and budgeting.
  • Provide transportation to outings, grocery shopping, and appointments.

Requirements

  • High school degree or equivalent.
  • Valid driver's license and reliable transportation with proof of current auto insurance.
  • Demonstrated professional history in networking and collaborating with community partners.
  • Strong skills in working with individuals with SMI diagnoses.
  • Ability to work independently and manage multiple responsibilities.
  • Professional oral and written communication skills.
