About the position

The Housing Navigator is a full-time role focused on providing community support services to adults with serious mental illness and substance use disorders. This position involves case management and care coordination to assist clients in recovery, ensuring they have access to necessary resources and support to live independently in the community. The Housing Navigator will work closely with community partners and clients to deliver flexible, individualized services, including outreach, skills training, and crisis intervention.

Responsibilities

  • Provides street outreach to homeless individuals.
  • Works with community partners to deliver services to homeless individuals.
  • Offers case management services and in-home oversight.
  • Educates clients in skills training.
  • Assists clients with personal financial administration.
  • Delivers appropriate therapeutic and crisis interventions.
  • Documents interventions according to department standards.
  • Initiates, monitors, and updates clients' treatment plans in collaboration with the treatment team.
  • Helps clients achieve their treatment goals.
  • Facilitates appropriate discharge planning and transition to aftercare services.
  • Completes electronic medical record documentation as required.
  • Plans and organizes group skills training experiences aligned with treatment plans.
  • Drives and transports clients as needed.
  • Enters data into the Homeless Management Information System (HMIS).
  • Submits required data for PATH Grant reporting in a timely manner.
  • Assists clients in applying for Social Security benefits through the SOAR program.
  • Connects clients to community resources and advocates for their needs.
  • Coordinates care with internal and external providers.
  • Creates group materials as needed.
  • Attends staff meetings, clinical supervision, and in-services.
  • Performs other duties as assigned.

Requirements

  • Bachelor's Degree in Social Work, Psychology, or a related human services discipline is required.
  • Previous related experience working in mental health or a related discipline is preferred.
  • Successful completion of OBHP educational requirements within 3 weeks of hire date.
