About The Position

The Multnomah County Health Department is seeking a visionary and dynamic leader to join our team as the Integrated Clinical Services Immediate Access Provider. This role is crucial in providing same-day access to new and established patients, thereby reducing referrals to external urgent cares and emergency departments. The Immediate Access Provider will work with a clinical FTE of 0.8 for direct patient care and 0.2 for indirect patient care time. As a newly hired Advanced Practice Clinician, you will receive a signing bonus of $2,500 and a retention bonus of $2,000 after one year of continuous employment. This position is integral to ensuring care continuity in the primary care setting and does not have a patient panel.

Requirements

  • Masters degree or equivalent in nursing from an accredited college or university.
  • Possession of a valid Nurse Practitioner license or Medical Doctor license.
  • CPR/BLS certification.
  • DEA certification.

Responsibilities

  • Perform comprehensive health assessments and clinical procedures, including physical and psychosocial history.
  • Examine, diagnose, treat, and advise patients on medical and behavioral health problems.
  • Prescribe and dispense medications; initiate and/or continue medical regimens for patients.
  • Perform treatments such as suturing, routine excisions, biopsies, and wound care.
  • Act as a representative on committees and provide consultation and education to partners.
  • Analyze and review laws, regulations, and policies to ensure compliance.
  • Seek clinical consultation from other clinicians as needed.
  • Review tests and develop care plans to ensure continuity with primary care.
  • Coordinate with primary care providers and other healthcare professionals.
  • Participate in continuing education to maintain licensure and current practice.
