About the position

This position is responsible for collaborating with healthcare team members to coordinate appropriate and cost-effective services for members of a defined population. The role focuses on enhancing the quality of life and achieving optimal independence for members through comprehensive psychosocial assessments and effective case management.

Responsibilities

• Collaborate with healthcare team members to facilitate coordination of services for members.
• Conduct comprehensive psychosocial assessments to evaluate patient goals and resources.
• Manage and coordinate assigned caseloads according to established criteria.
• Develop and monitor plans of care in collaboration with nurse case managers.
• Identify and advocate for appropriate organizational and community resources.
• Ensure reliable documentation of case management activities in compliance with standards.
• Analyze patient and program outcomes to identify improvements in case management activities.
• Facilitate applications for local, state, and federally funded programs.
• Provide supportive counseling and education to members and families.
• Maintain knowledge of laws and regulations related to social work practice.

Requirements

• Minimum one (1) year of clinical social work experience in a healthcare setting required.
• Masters degree in social work (MSW) required.
• Credentials from the primary work state are required before hire.

Nice-to-haves

• Experience with computer software programs in a Windows environment preferred.
• Knowledge of community systems and resources in the defined service area preferred.
• Knowledge of regulatory issues for the Mid-Atlantic area preferred.
