About the position

The position involves collaborating with healthcare team members to coordinate appropriate and cost-effective services for members of a defined population. The role focuses on facilitating care, enhancing quality of life, and promoting independence through comprehensive psychosocial assessments and resource management.

Responsibilities

  • Collaborate with healthcare team members to facilitate coordination of services.
  • Conduct problem identification and psychosocial assessments.
  • Provide financial counseling and referrals to community resources.
  • Guide members through health-related legal processes.
  • Manage and coordinate assigned caseload effectively.
  • Complete comprehensive psychosocial assessments to evaluate patient goals and resources.
  • Develop and monitor a plan of care in collaboration with the nurse case manager.
  • Identify and advocate for appropriate organizational and community resources.
  • Ensure reliable documentation of case management activities.
  • Analyze patient and program outcomes for improvements in quality and cost-effectiveness.
  • Facilitate applications for local, state, and federally funded programs.
  • Provide supportive counseling and education to members and families.
  • Maintain knowledge of laws and regulations related to social work practice.
  • Schedule and coordinate family meetings as needed.
  • Complete guardianship paperwork and assist with technology for court proceedings.

Requirements

  • Minimum one (1) year of clinical social work experience in a healthcare setting required.
  • Masters degree in social work (MSW) required.
  • Credentials from the primary work state are required before hire.
  • Licensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire.
  • Licensed Master Social Worker (Maryland) within 6 months of hire.
  • Licensed Master's Social Worker (Virginia) within 6 months of hire.

Nice-to-haves

  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
