About the position

The Licensed Physical Therapist Assistant (PTA) will support an innovative care coordination program within a senior living community. This role allows PTAs to practice at the top of their license while engaging in care coordination duties, providing an opportunity for career growth and flexible scheduling. The PTA will work closely with older adults, delivering physical therapy and wellness services under the supervision of a Physical Therapist (PT).

Responsibilities

• Providing physical therapy and wellness services for older adults under the supervision of a PT as part of a multidisciplinary team
• Supporting care coordination efforts by conducting regular wellness checkpoint visits with patients
• Communicating with patients' providers and caregivers
• Assisting patients with attending telehealth visits
• Maintaining professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
• Contributing to program development, quality improvement, and problem-solving
• Assisting with optimizing each patient's functional well-being and satisfaction

Requirements

• Completion of an accredited Physical Therapy Assistant program (New Grads Welcome)
• Current PTA state licensure
• Excellent verbal and written communication skills
