As a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN), you will join a mission-driven team that creates relationships that motivate us all every day. You will engage in high-impact work by delivering nursing services, training, and support to address the medical or psychiatric needs of individuals with brain injuries, intellectual and developmental disabilities, substance use disorders, and emotional or behavioral disabilities. In this position, you will monitor the health conditions of individuals served and ensure that quality services are provided. You will be responsible for administering medications, providing documentation, and overseeing staff members who administer medications. Additionally, you will provide prescribed medical treatment according to physician's orders and personal care services. Conducting assessments and implementing health care services to be included in each Individual Service Plan (ISP) will be part of your duties, as well as responding to changes in condition. You will also provide training to staff members on universal precautions, infection control, and medication administration. As a liaison and advocate, you will work with other medical and professional service providers and external agencies to coordinate services and ensure consistent treatment of individuals receiving care. Training for individuals served will include topics such as self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. This role is essential in ensuring that individuals receive the care and support they need to lead fulfilling lives.
