The Licensed Practical Nurse (LPN) for Home Health in Horry County is responsible for maintaining a professional image and exhibiting excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. The LPN will assist with revisits within a geographical area and will be supervised by the Team Clinical Manager at McLeod Home Health, working under the direction of the primary RN. The role involves collaboration with all caregiver staff, including team RN, PT, OT, ST, and MSW for interdisciplinary coordination of patient care. The LPN is accountable for independent actions consistent with professional nursing standards and must manage their time productively while complying with attendance and reporting requirements. Knowledge of State Licensure Laws, federal health insurance program conditions, and home health agency regulations is essential. The position requires strong communication skills, good interpersonal relationships, and the ability to exhibit good writing skills and computer literacy. The LPN must also demonstrate strong physical assessment and clinical skills, adaptability, initiative, and judgment in carrying out work assignments. Physical demands include moderate activity such as walking, standing, bending, and lifting objects weighing up to fifty pounds. The working conditions vary between an office setting and home visits, which may involve inclement conditions. The LPN must be familiar with growth and development principles and possess the ability to assess patient data reflective of their status. A high degree of maturity, self-motivation, and ethical professional behavior is required.
