About the position

The Licensed Social Worker (LSW) in Case Management at Fairfield Hospital is responsible for providing interventions and discharge planning services to patients and their families. This role facilitates smooth transitions for patients throughout their care continuum by utilizing hospital, community, and governmental resources, while addressing ethical issues and end-of-life considerations. This is a part-time PRN position requiring effective coordination with healthcare teams and advocacy for patient needs.

Responsibilities

  • Assess and allocate resources for patient management activities in hospital and community systems.
  • Complete initial and ongoing assessments of patients and families regarding their diagnosis and discharge planning needs.
  • Identify and report discharge barriers and avoidable days to promote resolution processes.
  • Coordinate patient and family interventions with healthcare providers and develop treatment plans based on assessments and goals.
  • Evaluate the effectiveness of interventions and revise discharge plans as necessary.
  • Collaborate with interdisciplinary teams to enhance quality of care and efficiency.
  • Maintain documentation that meets department and hospital standards for accountability and confidentiality.
  • Promote understanding and use of advance care planning among healthcare team members.

Requirements

  • Bachelor's Degree in Social Work required; Master's Degree preferred.
  • Licensed Social Worker (LSW) in the state of Ohio required.
  • Minimum of 1 year experience in an acute care setting.
  • Knowledge of casework principles, crisis intervention, and treatment planning.
  • Familiarity with community resources and public assistance systems, including Medicare/Medicaid.
  • Excellent verbal and written communication skills.

Nice-to-haves

  • Graduate Degree (Masters) in Social Work.
  • Experience with advance care planning processes.
