About the position

The LPN will work as a member of the Clinical Care Team at Oak Orchard Health, a leading patient-centered organization dedicated to improving access to quality health care for all. This role involves assisting medical providers in direct patient care and maintaining processes that ensure efficient, high-quality nursing and medical care. The LPN will be responsible for preparing patients for their visits, assisting providers in responding to patient needs related to medications, lab results, and preventative health measures. The position requires performing pre-visit planning, utilizing software tools to anticipate health maintenance and chronic care needs, and conducting huddles with care team members as necessary. In addition to patient preparation, the LPN will perform in-house labs and procedures in accordance with standard nursing practices. This includes administering vaccines and injections, managing medication refill requests, and providing nurse visits for patients. The LPN will also be responsible for maintaining accurate documentation in the electronic health record (EHR) system, adhering to infection control policies, and participating in quality assurance and performance improvement (QAPI) activities. The role requires active participation in nursing staff meetings and ongoing training to ensure compliance with organizational policies and procedures. Responsibilities

  • Maintains patient flow by preparing patients for their visits with providers.
  • Assists Providers in responding to patient needs related to medications, lab results, procedures, preventative/health maintenance measures.
  • Perform pre-visit planning utilizing software tools and chart review to anticipate health maintenance and chronic care needs.
  • Room/Prepare patients for provider visit including height/weight, vitals, med reconciliation, and verifying allergies.
  • Follow health maintenance and chronic care protocols and enter as appropriate into chart/alert provider/provide to patient.
  • Set up room/prepare patient as appropriate for procedures/examinations and chaperone visits as needed.
  • Perform in-house labs and procedures in accordance with standard nursing practice and/or health maintenance/chronic care protocols.
  • Prepare lab specimens to be sent to the lab.
  • Provide age and patient appropriate screenings and document/report as appropriate.
  • Review medication refill requests and check I-STOP (Prescription Monitoring System).
  • Administer vaccines, injections, and PPDs in accordance with standard nursing protocols and document appropriately.
  • Answer and document phone calls from patients and pharmacies in accordance with HIPAA regulations.
  • Provide nurse visits for patients and correctly reassign nurse visits to the appropriate on-site provider.
  • Clean exam rooms between each patient and maintain stock of exam rooms.
  • Preload new patient clinical chart information/history and complete paperwork as needed for Providers/patients.
  • Initiate and complete all parts of medication prior-authorization requests based on insurance requirements.
  • Perform family medicine/pediatric ED recalls and machine/POCT controls in accordance with manufacturer recommendations.
  • Document refrigerator/freezer temps and check expiration dates of injections, medications, samples, instruments, and supplies.
  • Sanitize instruments after use and maintain accurate documentation in EHR according to guidelines.
  • Adhere to center infection control policies and keep work area neat and tidy.
  • Actively participate in QAPI activities and maintain current Relias training.

Requirements

  • Graduate of an accredited program for Licensed Practical Nurse with a current valid license to practice nursing in New York State.
  • One year of experience in a nursing role preferred.
  • Ability to work with a diverse patient population.
  • Experience with EMR systems.
  • Excellent attention to detail.
  • Ability to communicate both written and verbally.
