The LPN will work as a member of the Clinical Care Team at Oak Orchard Health, a leading patient-centered organization dedicated to improving access to quality health care for all. This role involves assisting medical providers in direct patient care and maintaining processes that ensure efficient, high-quality nursing and medical care. The LPN will be responsible for preparing patients for their visits, assisting providers in responding to patient needs related to medications, lab results, and preventative health measures. The position requires performing pre-visit planning, utilizing software tools to anticipate health maintenance and chronic care needs, and conducting huddles with care team members as necessary. In addition to patient preparation, the LPN will perform in-house labs and procedures in accordance with standard nursing practices. This includes administering vaccines and injections, managing medication refill requests, and providing nurse visits for patients. The LPN will also be responsible for maintaining accurate documentation in the electronic health record (EHR) system, adhering to infection control policies, and participating in quality assurance and performance improvement (QAPI) activities. The role requires active participation in nursing staff meetings and ongoing training to ensure compliance with organizational policies and procedures.
